Program Guidelines
- MUST NOT HAVE DENTAL INSURANCE
- Participant fee must be paid before the first appointment
- Cannot be used as a Secondary Insurance
- All participant fees are non-refundable
- Participation is valid for one (1) year
- Limited Exams are not included in the plan and will be charged separately at a discounted rate
- Prepay $225 for the year
- Participants must pay an annual renewal fee on or before their anniversary date to receive ongoing benefits
- Restrictions apply with Care Credit
- Does not apply for office referrals, e.g., specialists
- Non-transferable between offices
